Your first step is going to be to find an ITIN mortgage lender; I'm not in a position or recommend or endorse any specific ones, but they can be easily found through an online search.
You've got plenty of home equity but your credit score may be a problem, particularly if it's below 580. There are lenders who will make such loans but expect to pay a steep interest rate. If you're looking to refinance, you may not be able to improve on the rate you presently have. You may be better off waiting a year or two until you can improve your credit score by keeping up with your current loan payments.
